Description

Job Summary: The Clinical Transplant Coordinator provides leadership and acts as an expert of knowledge and nursing practice to define and integrate practice models for designated transplant patient populations across the continuum of care. This role utilizes critical thinking and analysis along with clinical judgment to impact patient outcomes, functioning as an educator, consultant, researcher, manager, and collaborator. Primary duties and responsibilities: Advanced Clinical Practice and Assessment: 1. Conducts comprehensive health assessments and demonstrates a high level of autonomy and expert skill in assessing the complex responses of individuals and families to actual and potential health problems. 2. Administers direct patient care to meet the special needs of each age-specific patient population. 3. Assesses, analyzes and synthesizes clinical findings to result in optimum goal achievement for patients in a cost-effective manner. 4. Collaborates with the health care team to define and integrate practice models for designated transplant patient populations. 5. Coordinates transplants during the work day and after hours. 6. Under the direction of the physician, may assume 24-hour telephone management of pre- and post-transplant outpatients. Program Management and Policy Development: 1. Manages patient populations to preserve efficient, cost effective, quality care to both the patients and Emory Hospitals. 2. Identifies targets, assesses the situational variables and provides the appropriate situational leadership style to effect appropriate outcomes. 3. Develops policies and procedures for the delivery of care to the transplant population. 4. Collaborates with clinical pathway team and/or appropriate leadership and implements procedures to improve efficiency and effectiveness. 5. Provides transplant program information during negotiation of managed care transplant contracts. Education, Research, and Consultation: 1. Educates patients, families and nursing staff regarding transplant evaluation, discharge planning and ongoing education to promote health. 2. Functions as educator and consultant. 3. Acts as consultant to referring community health care specialists and primary care providers. 4. Participates in data collection for department research projects. Administrative and Professional Development: 1. Develops and maintains patient management systems (i.e.: transplant database, patient records, UNOS and CTRD). 2. Works with manager to formulate a plan for professional development. 3. Attends educational in-services as appropriate. 4. Participates in professional activities and organizations to maintain knowledge of current trends, practices, and developments. 5. Performs related responsibilities as required. Qualifications: Minimum required: Education - Valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board. Experience - Four years of recent clinical nursing experience as a Registered Nurse which includes at least two years in a specialty area. Licensure - Valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board. Certification - 1. BLS certifications. 2. If completing virtual care activities that may include multi-state practice, an active compact/multistate license (eNLC) is required within 60 days of hire. Employees in role prior to 11/09/2025 will have until their next renewal date to achieve compact status Physical Requirements: (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks. Environmental Factors: Factors affecting environment conditions may vary depending on the assigned work area and tasks. Environmental exposures include, but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock, Floor Surfaces, Hot/Cold Temperatures, Indoor/Outdoor conditions, Latex, Lighting, Patient care/handling injuries, Radiation, Shift work, Travel may be required. Use of personal protective equipment, including respirators, environmental conditions may vary depending on assigned work area and work tasks.
