We're seeking an Patient Flow Coordinator responsible for:supporting the Capacity Command Center in the day-to-day operations of the health system hospital transfer center and bed planning functions, ensuring exceptional customer service and quality care.
- Acts as staffing support to ensure bed planning and call acceptance at all times.
- Real time coordination of health system capacity management and referral acceptance to the most appropriate EHC operating unit. Manages real-time quality assurance reviews, escalates critical clinical issues related to hospital transfers and patient placement in real time.
- Responsible for real-time communication of current capacity status and Health System bed availability to Capacity Command Center leadership for dissemination to all EHC operational leaders and senior leadership.
- Coordinates, prioritizes, and optimizes patient flow using knowledge of clinical and operational activities throughout the hospital network.
- Patient Flow Coordination: - Collaborates with bed planning, physician, nursing leadership and other healthcare providers to resolve patient flow issues in real time. - Actively monitors Grand Central bed planning at all EHC OUs and the Transfer Center board to identify barriers to patient acceptance and flow.
- Communication: - Leads health system bed huddles, clearly articulating patient flow concerns and barriers for the OUs and identifying action items for the respective OUs to support efficient patient flow - Collaborates with house supervisors at each operating unit throughout the day to mitigate discharge and patient flow barriers. - Exhibit a high level of service excellence in all interactions with patients/customers/team members according to established behavioral standards.
- Resource Management: - Collaborates with bed planners and transfer center nurses for optimal bed utilization from a health system perspective. - Respond in real time to discharge delay notifications, discharge milestone overdue notifications and other discharge notifications sent from the electronic medical record.
- Escalation Management - Acts as first escalation point for the 24/7 call center managing all health system referral calls and patient placement activities. - Investigates and mitigates any immediate barrier to patient transfer acceptance to ensure the propagation of EHC reputation in the greater Atlanta area. - Immediately escalates any patient flow barriers that cannot be resolved within thirty minutes of onset. - Escalates to appropriate operating unit leader and/or command center leader to rectify barriers/issues that impact patient safety.
- Collaboration: - Work closely with EMS personnel, hospital staff, and other healthcare providers to ensure seamless patient transfers. - Collaborate with other departments throughout the operating units, to support overall operational efficiency. - Participate in team meetings, training sessions, and quality reviews.
MINIMUM QUALIFICATIONS: - Bachelors degree in nursing required.
- Must have a valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.
- Experience: Three years of recent (within the past five years) intensive care nursing experience that includes supervision and patient bed placement.
- Certifications: Emergency Medical Technician (EMT) certification preferred. Basic Cardiac Life Support Provider and ACLS certifications required.
